Your First Visit
Medical & Dental History
We'll review your complete medical and dental history to understand your overall health and any conditions that may affect your dental care.
Comprehensive Oral Exam
Our dentist will perform a thorough examination of your teeth, gums, and oral tissues to assess your current oral health and identify any areas of concern.
Digital X-Rays
We'll take digital X-rays of your teeth and jaw to detect issues that aren't visible during a visual exam, such as cavities between teeth or bone loss.
Treatment Planning
Based on our findings, we'll discuss your oral health status and any recommended treatments, answering all your questions and working with you to create a personalized care plan.
Patient Registration Form
To save time at your first appointment, please complete our patient registration form in advance and send it to us before your visit. You can fax, scan and email, or mail the completed form.
